It happens when your IP address changed between when you loaded the page to when you hit post, or search or whatever. It's a typical mark of a spammer or hacker trying to get around the security of the forum.
It happens on mobiles a lot, and I suppose is possible on shared computers that use a proxy like at a college or something. (I know the pain too) - but there is no way around it. If I turn it off, then the spammers will get in again
